Student Laboratory Incident Report Form

When an event occurs that causes harm to people or property, the information gleaned from the unintended outcomes should be used to help prevent future incidents. When a student is harmed in a laboratory incident, this must be reported to the supervising department and the Environmental Health and Safety (EHS) Department.

Faculty sponsors and/or instructors are required to immediately respond to a student's injury, illness, or exposure to a hazard. They must investigate, document, and report the circumstances using this form within 3 day.

The EHS Department can be reached at 594-7280 when prompt notification and requests for assistance are needed.
