Incident Report

This form should be used to report situations where students, visitors, and/or contractors are harmed and/or when property is damaged. It applies to incidents occurring in locations owned by the University, in University-operated facilities, and when University employees or agents are performing or participating in University-sponsored activities regardless of the location. Whenever possible, a Christopher Newport employee or representative should complete this form.  

Incidents involving injuries to employees should be reported using the Employee Incident Report Form found on the EHS interweb. 

The Environmental Health and Safety (EHS) Department manages the incident reporting and investigation processes. Thorough and accurate information reported in a timely manner is essential because it is required by external agencies.  The information may also help prevent future incidents and minimize risk to people, property, and operations.  

Please provide specific details, including location information, sequence of events, actions taken, observations, and contributing factors. Photographs of incident sites and/or damaged property are strongly encouraged and should be attached. A representative of the Environmental Health and Safety Department may contact you with additional requests for information or guidance on corrective actions.  Please reach out to Kate Miller at kaitlyn.miller@cnu.edu with any questions.
